read more › Patients with CTS most commonly experience tingling, numbness, weakness, or pain in the digits (thumb, index, middle, half of ring finger) innervated by the median nerve. Mild symptoms may include numbness and pain in the hand, wrist, and forearm. One may experience paresthesias, such as a "pins and needles" sensation, or as if one's hand is "falling asleep". Symptoms may cause you to wake up during the night, and may be reduced or relieved by shaking and moving the affected hand/wrist. Patients may notice times of clumsiness or a tendency to drop objects.

read more › CTS is diagnosed by carefully examining all the aspects of a patient that may be contributing to their condition. This includes a past medical and surgical history (see: About Carpal Tunnel Syndrome), current complaints and symptoms, and noting any associated social or occupational activities. These are then combined with physical exam findings and additional imaging. On physical exam, Tinel's sign and Phalen's Maneuver are the two classic tests associated with the diagnosis of CTS.(see below) In addition, a physician may test your two-point descrimination (looking for sensory loss) and hand strength, especially muscles of the thumb, to look for signs of muscle atrophy (shrinkage).

read more › An open carpal tunnel release is the traditional procedure and surgical method of repair for CTS. The open approach allows better visualization of the entire transverse carpal ligament. This outpatient minor procedure has an operative time that ranges from 15 to 45 minutes. The procedure is performed with local anesthetic to numb the hand and wrist. An incision is made over the transverse carpal tunnel on the wrist and palm of the hand. Careful dissection allows identification of the transverse carpal ligament, which is then cut under direct vision.

read more › Dr. Nguyen is proud to offer needle aponeurotomy as an office procedure for the treatment of Dupuytren's disease. Since 2003, he has helped many patients with this debilitating disease by means of this procedure. Needle aponeurotomy (also known as percutaneous needle fasciotomy) is a minimally invasive procedure to treat Dupuytren's disease. The procedure originated in France in the 1950's with limited availability in the United States until the early 2000's. It is important to mention that this procedure is an alternative to traditional open surgery (see: Other Treatment Options and Risks).

read more › In addition to needle aponeurotomy, several treatments for Dupuytren's disease are available to patients, usually dependent on the surgeon or physician being consulted. A variety of methods have been implemented to heal or close the wound afterwards (primary closure, left open to heal secondarily, skin grafting, local skin flap). This is performed in the operating room under general anesthesia. In addition to standard procedural risks (bleeding, bruising, swelling, damage to nerves/vessels), increased pain and wound healing problems (dehiscence) may occur secondary to a larger incision.
